COVID-19 Vaccines

Consult your family’s healthcare provider if you feel you have a condition that may preclude you from getting vaccinated. There are no more restrictions according to risk, and every adult and child over the age of five is eligible to be vaccinated.

Appointments are required before receiving a COVID-19 vaccine. Call your Family Medicine provider at the Platteville Clinic, call 608-348-4330 to schedule your vaccine shot or booster shot.

COVID-19 Testing

Appointments are required before any COVID-19 test.  Please call Family Medicine at Southwest Health at (608) 348-4330 ahead of time to schedule your COVID-19 test with your primary care provider. 

THIS IS A SAFE FACILITY

We continually monitor the COVID cases in our community and region and adjust our policies as needed. At this time, we have no restrictions on our patient or visitor policy.  

EMERGENCY AND URGENT CARE

Do not call 911 for general COVID-19 symptoms. Dial 911 only if you’re having severe difficulty breathing or have another life-threatening emergency.

Do not come to our Emergency Department or Urgent Care for COVID-19 testing or for vaccines. If you have a positive COVID test at home – DO NOT come in to get a test to see if you are truly positive – if your home test is positive – you are positive and need to quarantine.

PHARMACY: CURB-SIDE PICK-UP AND FREE DELIVERY

The Pharmacy at Southwest Health offers curbside pickup of prescriptions in addition to our standard free delivery service. You can avoid coming to the pharmacy to pick up your prescriptions. Call The Pharmacy at (608) 342-6200 with questions.
