Chloe Bainbridge, OD

Dr. Bainbridge aims to stay educated on the most current, evidence-based optometry practices to provide her patients with innovative and informative care. She believes medicine should be treated as a partnership between patient and provider. She treats her Eye Center patients with the same time, dedication, and commitment that she would invest in those closest to her. Dr. Bainbridge is a southwest Wisconsin native with a passion for ophthalmology and giving back to the community that gave so much to her.

Zachary Olsen,
OD, FAAO, FSLS

After working in Veterans Affairs for more than 9 years, Dr. Olsen is excited to be a part of the
personable health system that is Southwest Health. In the Eye Center, Dr. Olsen specializes in both optometry and ophthalmology, with a passion for ocular disease as well as comprehensive eye care. He enjoys seeing patients of all ages, educating and treating patients with the utmost care and compassion.

Thaddeus M. Wontor, OD

Dr. Wontor is a native of Wisconsin with a passion for making rural eye care great. Specializing in both optometry and ophthalmology, his main role in the Eye Center is primary eye care, including treating chronic conditions like diabetic retinopathy, glaucoma, and macular degeneration. It also includes treating emergent conditions like red eyes or ocular foreign bodies. He co-manages with many local ophthalmology surgeons for cataract surgery and fits a variety of contact lenses.

Nicole Klein,
OD, FAAO

Klein

Dr. Klein specializes in both ocular disease and ophthalmology as well as primary comprehensive eye care. She works closely with Eye Center patients involved in cataracts, glaucoma, and oculoplastic surgery. She enjoys caring for optometry patients of all ages.

Michael Tashner, OD

Dr. Tashner is a lifelong resident of southwest Wisconsin. In the Eye Center, he specializes in both optometry and ophthalmology, focusing on contact lenses, geriatric care, and provides ocular care to patients starting at one year of age, low vision, and co-management of medical and refractive surgery.

Mary Jo Oyen,
MD

Working in the Eye Center, Dr. Oyen specializes in both optometry and ophthalmology, with a particular interest in advanced surgeries. These include cataract surgery with advanced technology intraocular lenses and microinvasive glaucoma surgery. She also performs oculoplastic surgeries, including repairing droopy eyelids and inward or outward turning eyelids. She brings years of surgical experience with advanced technology to the community where she was raised.

Jenny Belken,
OD

In the Eye Center at Southwest Health, Dr. Belken specializes in diagnosing and treating ocular disease and ophthalmology. She enjoys getting to know her optometry patients and helping them address their visual needs. Dr. Belken is very knowledgeable regarding contact lenses, including specialty lenses.
