Chloe Bainbridge, OD
Dr. Bainbridge aims to stay educated on the most current, evidence-based optometry practices to provide her patients with innovative and informative care. After working in Veterans Affairs for more than 9 years, Dr. Olsen is excited to be a part of the
personable health system that is Southwest Health. In the Eye Center, Dr. Olsen specializes in both optometry and ophthalmology, with a passion for ocular disease as well as comprehensive eye care. He enjoys seeing patients of all ages, educating and treating patients with the utmost care and compassion.

Kylie has years of experience working in the family medicine clinic at Southwest Health. As a registered nurse before becoming an Advanced Practice Nurse Prescriber, Kylie understands how patient care and

Michelle Hauser, PA-C
Michelle has been a family medicine physician assistant for decades providing complementary and
primary care to patients of all ages. She has a deep commitment to rural healthcare and what it means for our community. Michelle has also spent time as a Community Education Coordinator and an Associate Faculty member at UW-Madison, where she developed a rural health curriculum for the Physician Assistant program. Katie Ponsler,

Katie, a lifelong resident of southwest Wisconsin, enjoys caring for patients throughout their lives. Her role in the ENT department is to help patients understand their health and work with them to develop a care plan. It’s important her patients have an active voice and feel confident in their decisions. Understanding how chronic sinus concerns, allergy, ear infections, sore throats, and hearing changes can affect her patients, Katie is enthusiastic about providing Ear, Nose, and Throat services to optimize patient outcomes.

In the Eye Center at Southwest Health, Dr. Belken specializes in diagnosing and treating ocular disease and ophthalmology. She enjoys getting to know her optometry patients and helping them address their visual needs. Dr. Belken is very knowledgeable regarding contact lenses, including specialty lenses.
Ann M. Lucas, MD
Dr. Lucas brings a wealth of experience in providing primary care for the whole family through her
