Social Worker

Requirements

  • Bachelor’s degree in Social Work, or related field with a certificate in social work required, Master’s Degree preferred
  • One to two years’ experience required
  • Must be licensed as a Social Worker in the State of Wisconsin.

Work Hours

We are seeking a full-time Social Worker to work Monday- Friday, 8:00 am- 4:30pm.

Job Summary

The hospital Social Worker will coordinate activities that promote quality outcomes, patient throughput and discharge planning while supporting a balance of optimal care and appropriate resource utilization. The Social Worker will identify potential barriers to patient throughput and quality outcomes minimizing delays in discharge plans.
