Mission Accomplished!

The sun is setting on our last day of our 100 Miles in 100 Days Community Walking Challenge in 2021. The final day (and our FINALE event) is Tuesday, September 14.

On September 14 or after, fill out the form below to:

a) verify your amazing accomplishment

2) record or update your t-shirt preference

Or click here to print a paper copy you can fill out and return to Southwest Health. You can also complete this form at the FINALE event on Tuesday, September 14th at Katie’s Garden at the Platteville Regional Chamber. The FINALE begins at 5:15 pm. We will take some time to fill out forms, congratulate one another, and then walk our final mile together!

T-shirts will not be available immediately. With completed forms, we will order your free t-shirts and make plans for you to pick them up. They will be available to you at both our Sole Mates Monday evening walks or by stopping at the hospital. We will announce via email and via Facebook when they will be available for pick up.
