Requirements

  • Minimum requirements of A Bachelor’s degree in clinical Exercise Physiology or Kinesiology with a rehabilitation experience.
  • A Master’s Degree in Exercise Physiology is preferred. Or be a graduate of an accredited nursing school and have a valid Wisconsin registered nurse license with rehabilitation experience.
  • Minimum of two years of experience with telemetry, cardio/pulmonary rehabilitation is preferred.
  • Basic Life Support (BLS) certification required or able to complete immediately after hire. Advanced Cardiac Life Support (ACLS) certification required within 6 months of hire.
  • Highly encouraged to be member of state and national rehabilitation associations.
  • Basic proficiency in word processing and Excel programs; software proficiency covering a variety of health care/clinical record applications.

Work Hours

We are seeking a full-time (36 hours/week) Cardiopulmonary Rehab Coordinator to work Monday-Friday (8:00am-4:00pm).

Job Summary

This position is responsible for coordinating an inter-disciplinary treatment plan to meet the clinical and educational needs of both cardiac and pulmonary patients and in families in all phases of the rehabilitation program. The program uses a case management model for patient care. As a result, the coordinator must have the ability to independently plan and organize his/her daily work schedule, keeping patient care and safety a priority. A wide variety of internal and external professional relationships are involved on performance of the duties of the position. The position requires the ability to interact with internal customers at all levels. i.e. medical, clerical and administrative. External contacts necessary in this position include outside referring physicians, patients, families and other facilities involved with the referrals of patient in need of Cardiac or Pulmonary Rehab.
