| Requirements |

Southwest Health Center's long term care facility in Cuba City, WI is accepting applications for the following shifts:
Part-time days (6:00 a.m. to 2:30 p.m.)
Full and part-time evenings ( 3:30 p.m. to 11:00 p.m.)
Full-time (36 hrs/wk) and part-time ( 20 hrs/wk) Nights (11:00 p.m. to 7:00 a.m.)
Work every other weekend required with all positions. |
| Job Summary |
The CNA assists residents with activities of daily living, provides basic nursing care to residents, and assists in maintenance of a safe and clean environment under the direction and supervision of the RN and/or LPN in charge of the team and/or unit. The CNA assists in the implementation and coordination of care based on care plans and nursing diagnoses by providing delegated aspects of care. |
