Senior Services

Southwest Nursing Home
Once of the most important benefits of nursing homes is easy access to health care. Southwest Health Center makes available to you a wide range of professional medical services, including family physicians and extensive specialty, diagnostic, and therapeutic support services.

Senior Behavioral Sciences
We've moved to 1185 North Elm Street in Platteville, WI. Click here to see our new Park Place campus.

Senior Behavioral Sciences provides a warm, gentle environment for the individualized needs of an older person. We recognize that people over the age of 55 are dealing with issues that are very different from those in their 20s, 30s and 40s. Many are dealing with the loss of a spouse, loved ones, or friends. They may have limited mobility, chronic health problems, or illnesses such as Alzheimer's disease or Parkinson's disease, which alter their ability to think clearly.

Senior Support Services

Caregiver Care
Caregivers need caring as well. Just as workers need time off from their jobs, caregivers need time away from those they care for in order to recoup and regroup. We invite area residents who provide care to others to contact us about providing short-term care for those you care for. This service, sometimes referred to as respite care, is what we refer to as Caregiver Care.

Caregiver Care is an inexpensive opportunity for caregivers to get a much-needed break while our experienced and dedicated staff tends to a loved one. With a physician's approval, Southwest Health Center will provide care for a loved one from as little as 24 hours up to 28 days.

While in our program, your loved one - our guest - receives home-cooked, dietician-managed meals, activities and recreational opportunities, and skilled nursing supervision, all in a comfortable setting. Guests are invited to bring personal items with them, such as a small portable television, photographs, toiletries, and their own attire. Contact our Social Services Department at (608) 744-3958.

Meals At Home
Our meals at home program brings meals to your home weekdays for a nominal fee. Contact our Food Services Department at (608) 744-3957.

Arthritis Exercise Program
This exercise program provides gentle motion and activities to help increase joint flexibility, increase range of motion, and maintain muscle strength. This program is instructed by a certified Instructor and held at the hospital in Platteville, and it is sponsored by Southwest Health Center and the Arthritis Foundation. Contact our Rehabilitation Department at (608) 342-5081, or see our on-line Calendar of Events.

Mood & Memory Screenings
Often mistaken as signs of "just getting old", the signs and symptoms of mood changes and memory loss can be debilitating. Confidential assessments are available by contacting us at (608) 348-3656.
