Directions FROM THE EAST (Madison, WI): Take Highway 151 heading west. Take the Highway 80/81 exit (exit #19) heading north. The first exit on your right will be Enterprise Drive. (This is directly across the road from Farm and Fleet.) Take Enterprise Drive about ¼ mile and the road will veer to the left. At that point you will be on East Side Road. You will see Southwest Health Center on your left.

Direction FROM THE WEST (Dubuque, IA): Take Highway 151 heading east. Take the Highway 80/81 exit (exit #19) heading north. The first exit on your right will be Enterprise Drive. (This is directly across the road from Farm and Fleet.) Go about ¼ mile and the road will veer to the left. At that point you will be on East Side Road. You will see Southwest Health Center on your left.

Directions FROM THE NORTH (Potosi, Livingston, Lancaster, WI): Take highway 80/81 all the way through Platteville. Just before you get to the Highway 151 exit, take a left onto Enterprise Drive. (This is directly across the road from Farm and Fleet). Go about ¼ mile and the road will veer to the left. At this point you will be on East Side Road. You will see Southwest Health Center on your left.
